New Guidelines for School Immunizations 2014-2015

Georgia’s Immunization requirements for children have been revised based on the recommendations of the Advisory Committee on Immunization Practices(ACIP) beginning in the 2014-2015 school year.

If you child is entering Kindergarten they will need a complete and up to date 3231 form for registration. All children must have their 4 year old shots before the form can be completed.

Due to these new requirements all children entering the 7th grade after 07/01/2014 will be required to have additional vaccines. These vaccines are TDAP and MENINGITIS. If your child has not had the required 2nd VARICELLA(chickenpox) vaccine they will need that as well.

The schools are audited every year for children entering kindergarten and 7th grade. If your child does not have a current and up to date 3231 form they may not be able to return to school until this is completed and returned to the school nurse. A religious exemption form is required for all students whose parents opt out of vaccinating children and must be signed and notarized to be complete.
